Hospice Admissions Registered Nurse Per Diem

Position Summary
Seeking an experienced and compassionate RN to provide per diem hospice visits and assist with intake, hospice evaluations, and admissions. The RN will assess prospective patients for hospice appropriateness, review clinical information, explain hospice services and benefits to patients and families, and communicate findings to the hospice physician and clinical team.
Responsibilities
- Perform hospice RN visits and comprehensive nursing assessments.
- Assess and manage pain, shortness of breath, wounds, anxiety, agitation, and other end-of-life symptoms.
- Educate patients and caregivers regarding medications, symptom management, safety, and disease progression.
- Review referrals, medical records, diagnoses, functional decline, nutritional status, and other indicators of hospice eligibility.
- Conduct hospice informational and evaluation visits.
- Explain hospice philosophy, Medicare Hospice Benefit, covered services, levels of care, and the admission process.
- Discuss goals of care and answer patient and family questions.
- Communicate clinical findings to the hospice physician and DPCS/DON for eligibility determination.
- Complete hospice admissions and coordinate medications, DME, supplies, and interdisciplinary services.
- Identify patients requiring urgent follow-up or increased symptom management.
- Complete accurate and timely documentation.
Qualifications
- Current unrestricted RN license.
- Hospice, palliative care, home health, geriatrics, or similar experience preferred.
- Knowledge of hospice eligibility and Medicare hospice guidelines.
- Strong assessment, critical-thinking, and communication skills.
- Comfortable discussing hospice and end-of-life care with patients and families.
- Able to work independently and communicate effectively with physicians, facilities, and referral sources.
- Reliable transportation, valid driver's license, and automobile insurance.
Schedule
Per diem/PRN based on agency needs, including hospice visits, evaluations, informational visits, and admissions. Some evening or weekend availability may be requested.
Compensation: $100.00 - $300.00 per hour
